JUNE has not been a good month for Stephen Harper, the Canadian prime minister. At the G8 summit in Germany his record on foreign aid was singled out for special criticism by two Irish rock stars with planet-saving ambitions, Bono and Bob Geldof. His return to Canada was blighted by the loss of a Conservative MP, bounced from the party's caucus for opposing the budget, and by a confrontation caught on camera with an 80-year-old widow, who accused him of lying about extending veterans' benefits. The government's much-vaunted green plan was denounced as ineffectual by a prominent think-tank usually sympathetic to the Conservative cause. And as Parliament limped toward its summer recess this week, two angry premiers descended on Ottawa to castigate the prime minister for supposedly breaking his word to them on financial transfers to their provinces. It is a far cry from six months ago, when Mr Harper was riding high in the polls and appeared poised to call an election that would turn his governing Conservative minority into a majority. After one year in power, he had largely kept five narrow campaign promises and could convincingly argue the need for a new mandate. But the chance disappeared as public support for the Conservatives, which peaked after the launch of a generous budget in March, quickly dropped back into minority territory, where it remains. Such a litany of woes might be expected to worry a party that holds only 124 of the 308 seats in the House of Commons there are 100 Liberals, 49 Bloc Qubcois, 29 New Democrats, four independents and two vacant seats ; . Altered Cortical Pain Processing in Chronic Pain Patients L. VOSS and J. BARNARD Waikato Clinical School, University of Auckland, Hamilton, New Zealand Chronic pain is associated with neural plasticity at multiple levels in the nervous system. In this study we looked for evidence of altered cortical functioning through analysis of the EEG during repetitive sensory simulation of a finger. A group of patients with chronic pain was compared to a group of normal volunteers. The EEG of each subject was continuously recorded using a 28-channel montage. Brief repetitive at least 60 ; electric shocks were given via an electrode on the dominant index finger. Two intensities of electric shock were tailored to each subject, one that was easily felt but not painful and one that was moderately painful. The shocks were given in three sequences: 1 ; non-painful shocks, 2 ; painful shocks, 3 ; a random sequence of non-painful and painful shocks 4: 1 ratio ; . The raw EEG was cleaned and 0.5second post-stimulus epochs averaged for each sequence giving clear sensory evoked potentials SEPs ; . The SEPs were then analysed in greater detail. The most striking feature was the negative postive negative wave starting at approximately 200ms over the vertex. An increased amplitude was seen with painful stimuli versus non-painful stimuli. The wave was delayed by 20msec in the chronic pain patients versus the normal volunteers p 0.05 ; . In conclusion, detailed analysis of SEPs from multi-channel EEG demonstrates differences between the painful and non-painful stimuli and provides evidence of altered cortical processing in patients with chronic pain compared to normal volunteers.

Studies that document pharmacokinetics of triptans in children and adolescents were included if age, dosage, and adverse effects were adequately reported. A literature search was conducted to assemble all published studies English or French ; . Search terms included: migraine, triptan. Additional reports were sought from the reference lists of the retrieved studies. The last search was completed in November 2002. Eligibility was determined by reading the abstract of each report identified in the search. Papers that met the inclusion criteria based on their abstract were reviewed in detail by the authors, and agreement was reached by discussion. The reports were not made anonymous in any way before assessment. Literature search results are summarized in Table 1. Four randomized controlled trials were considered eligible. One study used oral sumatriptan, another oral rizatriptan, and two studies used nasal spray sumatriptan. Four open label, prospective studies were also included to study safety. Two used subcutaneous sumatriptan, one used nasal spray sumatriptan, and one used oral zolmitriptan.
