PREPARED STATEMENT OF HON. JAMES C. GREENWOOD, CHAIRMAN, SUBCOMMITTEE ON OVERSIGHT AND INVESTIGATIONS This morning the Subcommittee examines the heart-wrenching and very complicated issues relating to the safety of Accutane, a tremendously powerful drug approved for treating severe, recalcitrant, cystic acne. Manufactured by Hoffman LaRoche, Accu5ane is the only drug that has the potential to clear severe acne that is resistant to standard treatment. Although the severe lesions Acccutane treats are not life-threatening, patients--as a medical reviewer at the Food and Drug Administration FDA ; has noted--``know all too well the very real suffering caused by this disfiguring disease.'' Yet as powerful as Acxutane is in relieving suffering and improving lives, its adverse effects can be just as powerful and have required extraordinary vigilance over the years. Just like the sleeping pill thalidomide, linked 40 years ago to gross birth deformities in Europe, Accuttane is a teratogen. It is known to cause miscarriages.
Effects on triglycerides, HDL, and cholesterol were reversible upon cessation of Acctane therapy. Some patients have been able to reverse triglyceride elevation by reduction in weight, restriction of dietary fat and alcohol, and reduction in dose while continuing Accutane.5 Blood lipid determinations should be performed before Accutane is given and then at intervals until the lipid response to Accutane is established, which usually occurs within 4 weeks. Especially careful consideration must be given to risk benefit for patients who may be at high risk during Accutane therapy patients with diabetes, obesity, increased alcohol intake, lipid metabolism disorder or familial history of lipid metabolism disorder ; . If Accutane therapy is instituted, more frequent checks of serum values for lipids and or blood sugar are recommended see PRECAUTIONS: Laboratory Tests ; . The cardiovascular consequences of hypertriglyceridemia associated with Accutane are unknown. Animal Studies: In rats given 8 or 32 mg kg day of isotretinoin 1.3 to 5.3 times the recommended clinical dose of 1.0 mg kg day after normalization for total body surface area ; for 18 months or longer, the incidences of focal calcification, fibrosis and inflammation of the myocardium, calcification of coronary, pulmonary and mesenteric arteries, and metastatic calcification of the gastric mucosa were greater than in control rats of similar age. Focal endocardial and myocardial calcifications associated with calcification of the coronary arteries were observed in two dogs after approximately 6 to 7 months of treatment with isotretinoin at a dosage of 60 to 120 mg kg day 30 to 60 times the recommended clinical dose of 1.0 mg kg day, respectively, after normalization for total body surface area ; . Hearing Impairment Impaired hearing has been reported in patients taking Accutane; in some cases, the hearing impairment has been reported to persist after therapy has been discontinued. Mechanism s ; and causality for this event have not been established. Patients who experience tinnitus or hearing impairment should discontinue Accutane treatment and be referred for specialized care for further evaluation see ADVERSE REACTIONS: Special Senses ; . Hepatotoxicity Clinical hepatitis considered to be possibly or probably related to Accutane therapy has been reported. Additionally, mild to moderate elevations of liver enzymes have been observed in approximately 15% of individuals treated during clinical trials, some of which normalized with dosage reduction or continued administration of the drug. If normalization does not readily occur or if hepatitis is suspected during treatment with Accutane, the drug should be discontinued and the etiology further investigated. Inflammatory Bowel Disease Accutane has been associated with inflammatory bowel disease including regional ileitis ; in patients without a prior history of intestinal disorders. In some instances, symptoms have been reported to persist after Accutane treatment has been stopped. Patients experiencing abdominal pain, rectal bleeding or severe diarrhea should discontinue Accutane immediately see ADVERSE REACTIONS: Gastrointestinal ; . Skeletal Bone Mineral Density Effects of multiple courses of Accutane on the developing musculoskeletal system are unknown. There is some evidence that long-term, high-dose, or multiple courses of therapy with isotretinoin have more of an effect than a single course of therapy on the musculoskeletal system. The combination of events which led to this very unfortunate incident should be viewed by all pharmacists as a warning of the serious results that can occur when they are not diligent in maintaining standards of practice, and do not take the time to ask the questions that should be asked, or take the time to counsel patients adequately. In this particular incident, a number of factors combined in a chain of events that ended in tragic results. These factors include: possible distractions at the time of a shift change, failure to clarify an unclear request from the patient, failure to follow explicit transfer requirements, failure to recognize the value of inactivating older prescriptions that are superseded by newer prescriptions, incomplete patient profiles because the patient went to more than one pharmacy, failure of the prescription checking process and inadequate pharmacist patient dialogue. This incident should cause all pharmacists to re-examine the procedures they use when giving or receiving transfers of prescriptions, their policy with respect to inactivating superseded prescriptions, the diligence of their prescription checking process and most importantly their commitment to adequate patient dialogue and counselling. All pharmacists should ask them selves: "Could the cautions given by the panel apply to me, as well as to Pharmacists A, B and C?" "Are there lessons that can be learned from this tragic incident that can help me and my pharmacy prevent another similar incident from happening?" Carcinogenesis, Mutagenesis and Impairment of Fertility In male and female Fischer 344 rats given oral isotretinoin at dosages of 8 or mg kg day 1.3 to 5.3 times the recommended clinical dose of 1.0 mg kg day, respectively, after normalization for total body surface area ; for greater than 18 months, there was a dose-related increased incidence of pheochromocytoma relative to controls. The incidence of adrenal medullary hyperplasia was also increased at the higher dosage in both sexes. In general, there was microscopic evidence for appreciable depression of spermatogenesis but some sperm were observed in all testes examined and in no instance were completely atrophic tubules seen. In studies of 66 men, 30 of whom were patients with nodular acne under treatment with oral isotretinoin, no significant changes were noted in the count or motility of spermatozoa in the ejaculate. In a study of 50 men ages 17 to 32 years ; receiving Accutane isotretinoin ; therapy for nodular acne, no significant effects were seen on ejaculate volume, sperm count, total sperm motility, morphology or seminal plasma fructose and acticin.

However, HES can be fatal, particularly when eosinophils infiltrate vital organs. We report a patient with HES who presented with a sepsis-like syndrome and rapidly improved with Drotrecogin-alpha therapy. CASE PRESENTATION: A 52 yr old male presented to an outside hospital OSH ; with debilitating abdominal pain 12 days prior to his transfer to Indiana University. Two months ago he underwent an outpatient evaluation for fatigue, arthralgias, nonproductive cough and intermittent abdominal pain. Upon admission to the OSH he underwent an exploratory laprotomy which revealed colonic perforation. A small portion of bowel was excised with end to end anastomosis. Surgery went well but he was reintubated 2 days later for increasing dyspnea. Upon transfer, the patient was intubated, on FiO2 of 100% and PEEP of 15. He was hypotensive and required levophed. Except for a heart rate of 122, a benign post op abdomen, and bilateral rhonchi his exam was normal. Pregnancy After Accutane Therapy The terminal elimination half-life of Accutane varies but is generally within 10 to 20 hours. The elimination half-life of one of the isotretinoin metabolites, 4-oxoisotretinoin, is approximately 25 hours. Since plasma elimination is host dependent, prescribers should warn patients not to become pregnant for 1 month posttreatment. Women who become pregnant during this month should be counseled as to the outcome data. In 1989, Dai et al1 reported the results of an epidemiologic study of pregnancies that occurred in women who conceived after discontinuing Accutane. They studied women from 5 days to more than 60 days between their last dose of isotretinoin and conception. The incidence of birth defects in former Accutane patients was not significantly different from the rate in the general population. Accutane is found in the semen of male patients taking Accutane, but the amount delivered to a female partner would be about 1 million times lower than an oral dose of 40 mg.
